• The basic details of your condition and age during treatment.
• A brief description of your physical therapy experience (including your condition before physical therapy and afterward).
• A statement that describes what you want others with your symptoms or condition to know about what physical therapy did for you.  
• The name and location (city and state) of the clinic where you received treatment and your physical therapist's name.

Those selected to participate will be contacted within two weeks of their submission. Selected stories may be shared on webpages at ChoosePT.com, via social media, podcasts, or videos.
