Practice Description: Blake Outpatient Therapy is located just north of Blake Medical Center. We offer PT, OT, and ST with multiple specialties and certifications: Orthopedic Certified Specialists, Certified Hand Therapist, Certified Lymphedema Therapists, LSVT BIG and LSVT LOUD Certifications, Interactive Metronome, and Certificate of Competency in Vestibular Rehabilitation. In addition, we offer Aquatic Therapy in a heated swimming pool. We accept most insurances, but a Florida physician referral is required.
