Practice Description: Currently, I work at LeRoy Viillage Green Nursing Home outside of Rochester, NY. My job involves working with geriatric patients in the sub-acute rehabilitation and long term care settings. My responsibilities include facilitating a safe transition back home for patients after a hospital stay and also to maximize the independence of residents who live at our facility.
